Community Outreach Nurse
Carolina Collaborative Community Care (4C) is seeking a part time, (10 hours/week maximum), Community Outreach Nurse to work on a PRN basis. The Community Outreach Nurse will represent the organization at designated community-based events. (i.e. Dogwood Festival, Food/Clothing drive, etc.)
Job Description:
The individual will also work with patients per contracted programs and participating physicians contracted with 4C. The nurse will collaborate with providers to develop, implement and prioritize quality improvement and care management initiatives targeting the patient population(s) of assigned contracted program(s) and community-based programs.
The individual will also identify patients who would benefit from disease management and case management services; educate and engage patients in their own care management; and link patients to appropriate levels of care/appropriate and supportive services;
Requirements for Hire:
- Associate or Bachelor Degree in nursing with a current LPN or RN license required; CCM preferred.
- Must have a valid driver’s license and an automobile for use during work.
- Strong organizational and time management skills, and the ability to work independently are required.
Carolina Collaborative Community Care seeking an experienced RN Care Manager.
The nurse will provide care management with focus on our beneficiaries with behavioral health needs in a designated coverage area of Cumberland County and surrounding counties. The care manager objectives are:

Experience in assessments, behavioral health care management, and commitment to high performance are a must.
Responsibilities
-Develop, assess, and coordinate holistic care management activities, with primary focus and support towards populations with mental/behavioral health needs, to enable quality, cost-effective healthcare outcomes.
-Support psychotropic medication management prescribed by providers, focusing on treatment adherence, side effects and other complications, and effectiveness treatment.
Support beneficiaries with primarily mental/behavioral health needs, such as those with (or a history of) depression, anxiety, bipolar disorders, schizophrenia, substance use disorder, post-traumatic stress disorder, borderline personality disorder, etc.
-Work with patients who in addition to a behavioral health diagnosis, also have medical comorbidities
such as diabetes, hypertension, asthma, sickle cell, and hyperlipidemia and helps patients meet treatment goals.
Requirements
- BSN or RN
- Licensure in the state of North Carolina
- Greater than 2 years Clinical/Utilization Management/Discharge Planning/Home Care/Social Work/Ambulatory experience
- Strong organizational skills with attention to detail to ensure compliance with healthcare regulations.
- Proficient in Microsoft Office and keyboarding
RN Care Manager
